    Despite major scientific advances, heart failure continues to be a common and costly condition, and each year more than 1 million people are admitted to an inpatient setting for acute heart failure. There is an urgent need for low-cost solutions to reduce morbidity in heart failure.  Several studies have shown that self-monitoring can improve quality of life in heart failure patients. Self-monitoring includes daily tracking of symptoms, weight, and exercise. Accurate self-monitoring, feedback and self-efficacy (an individualís belief in their ability to perform a specific behavior) are essential components of the self-regulation cycle and are critical for managing heart failure.  ManageHF4Life is a mobile application designed to promote self-monitoring and self-management of heart failure. Behavior change interventions that focus on self-regulation are particularly well suited for automation. The use of technology can decrease the burden of self-monitoring and enhance self-regulatory efforts. Cyclical processes of goal setting, behavior change, and goal assessment can be easily tracked, charted, reviewed and automated. (01/01/2017 - 01/31/2019)

    Thanks to new technology, prospective data collection using the interactive voice response (IVR) system and text-messaging (TM) has become possible in recent years. These new measurements have the advantages of cutting costs of staff time as well as minimizing recall bias and tendency to underreport socially undesirable behaviors. The long-term goal of this project is to abstract crucial information from daily process data of health risk behaviors to inform prevention and intervention. The overall objective of this application, which is the next step toward attainment of our long-term goal, is to develop cost-effective measurement and cutting-edge methodology to collect and analyze daily process data.     The urban emergency department (ED) represents an underutilized venue for delivering violence interventions among adolescents. In the United States, there are over 100 million ED visits each year, of which at least 3 million are the result of violence. A recent study (the SafERteens Study) demonstrated the efficacy of an ED-based BI for violence on changing attitudes, self-efficacy, and reducing violent behaviors, peer victimization, and violence related consequences. From a public health standpoint, however, in order to reduce violence on a broader scale studies are needed to determine the effectiveness of the SafERteens behavioral intervention (BI) when delivered by clinical staff in real world ED settings. This study is designed to translate this efficacious BI for violence into a practical prevention program incorporated into ED clinical practice; with ED staff conducting the screening and BI. Specifically, we will determine the reach, effectiveness, adoption, implementation, and maintenance of the SafERteens BI in two diverse and novel ED settings: Childrenís Hospital of Philadelphia (CHOP) and Grady Memorial Hospital in Atlanta (GMH).     The purpose of this project is to develop and test a web-delivered, tailored decision aid for women at risk for breast and ovarian cancer. Existing educational materials are refined then offered in an interactive website featuring greater personalization of risk information, using validated risk communication techniques in a patient-friendly Web-based application.Such technologies are needed to expand the reach and improve the cost- effectiveness of breast cancer genetic services and are part of a growing movement within clinical care to provide validated patient decision aids.   • Stepping Up to Health - Expanding the Reach

    The goal of this project is to expand the web-based Stepping Up to Health intervention to more individuals. The original intervention used enhanced pedometers and email-based tailored feedback to promote physical activity in people with type 2 diabetes. Expanding the Reach extends the tailored messages to promote physical activity to sedentary adults who are at risk for developing cancer, heart disease or diabetes, along with continuing to provide tailored messages for individuals with type 2 diabetes.   • Comorbid Psychopathology - Teen Smoking Cessation

    Adolescent cigarette smokers have disproportionately high rates of co-occurring psychiatric and substance use disorders, similar to those found in the adult smoking population. In the absence of intervention, adolescent smokers with co-morbid psychopathology are likely to become highly dependent, recalcitrant adult smokers who have extreme difficulty quitting smoking. The overall objective of this research program is to develop effective smoking cessation approaches for these high risk youth, and to advance the applicant's knowledge of the relationship between psychopathology and smoking cessation within the context of a treatment outcome study.
